T2 values of the acutely infarcted myocardium following primary PCI: the relationship with infarct characteristics and gadolinium extracellular volume of distribution

چکیده
Background T2 mapping has been shown to detect myocardial oedema as assessed by histology in animals, and is a promising sequence to detect the area at risk in STEMI. The specific cut off value to identify abnormal myocardium in STEMI has not been identified nor have the effects of infarct characteristics on T2 valves. The extra cellular volume fraction (ECV) has been validated against histology in humans. The aims of this study were to identify a cut-off T2 for prediction of myocardial oedema following acute reperfused myocardial infarction and determine if infarct characteristics affect T2 values. In addition, this study aims to determine if T2 values are associated with the ECV following STEMI.

Extra-cellular expansion in the normal, non-infarcted myocardium is associated with worsening of regional myocardial function after acute myocardial infarction
BACKGROUND Expansion of the myocardial extracellular volume (ECV) is a surrogate measure of focal/diffuse fibrosis and is an independent marker of prognosis in chronic heart disease. Changes in ECV may also occur after myocardial infarction, acutely because of oedema and in convalescence as part of ventricular remodelling. The objective of this study was to investigate changes in the pattern of...
متن کاملDetermination of the area at risk using quantitative T2 mapping in re-perfused myocardial infarction; a comparison with late gadolinium enhancement and CINE imaging
Methods Between Feb.2012, June.2012, 15 patients with acute MI underwent cardiac MRI after percutaneous primary coronary intervention (PPCI). Acute MI was diagnosed, based on EKG, coronary angiography and cardiac biomarker. In short axis view, every analyzed myocardium was subdivided into 30 sectors evenly using commercially available software (Argus; Siemens Medical Solutions, Erlangen, German...

Methods Twenty patients underwent CMR at 3T (Achieva TX system, Philips Healthcare, Best, The Netherlands) within 3 days following reperfused ST-elevation MI. Black blood, cine, T2w, T2-mapping, T2*-mapping and LGE imaging (0.1mmol/kg gadolinium DTPA) were performed in identical short axis locations using the ‘3 of 5’ approach. Data were evaluated offline using commercial software (cvi42 v4.1.5...
متن کاملMyocardial Extracellular Volume Estimation by CMR Predicts Functional Recovery Following Acute MI
OBJECTIVES In the setting of reperfused acute myocardial infarction (AMI), the authors sought to compare prediction of contractile recovery by infarct extracellular volume (ECV), as measured by T1-mapping cardiac magnetic resonance (CMR), with late gadolinium enhancement (LGE) transmural extent. BACKGROUND The transmural extent of myocardial infarction as assessed by LGE CMR is a strong predi...
متن کاملMechanisms for overestimating acute myocardial infarct size with gadolinium-enhanced cardiovascular magnetic resonance imaging in humans: a quantitative and kinetic study†
AIMS It remains controversial whether cardiovascular magnetic resonance imaging with gadolinium only enhances acutely infarcted or also salvaged myocardium. We hypothesized that enhancement of salvaged myocardium may be due to altered extracellular volume (ECV) and contrast kinetics compared with normal and infarcted myocardium. If so, these mechanisms could contribute to overestimation of acut...
